Funding Opportunity: Expansion of Medicaid for School-Based Health Services

Eligible applicants are State Medicaid Agencies from all 50 states, Washington, D.C., and U.S. territories. Eligible U.S. territories include American Samoa, Guam, the Northern Mariana Islands, Puerto Rico, and the United States Virgin Islands. Only one application can be submitted for a given State Medicaid Agency. The term “State Medicaid Agency” means the single State agency for medical assistance provided under Title XIX of the Act. A Tribal organization, if interested, must come under the auspices of and work with the State Medicaid Agency in the implementation of this demonstration program. The State Medicaid Agency or designee must be the recipient and oversee implementation of the grant; however other entities may apply in conjunction as subrecipient(s) including any other State agencies and/or Tribes, or Tribal organizations providing SBS through the State Medicaid or CHIP program.
